The application for business licensing includes a “Verification of Eligibility” form that must be sent to the Board office for verification, and will be returned to you for submission with your business license application to the SOS office.

The Board does not regulate patient record retention; however, the general rule of thumb is 10 years. Be sure to contact the patient’s insurance provider for more information.
Will the Board consider counting live-stream continuing education as “in-person” hours?
