Applying for Licensure

WV Licensure

​Persons licensed to practice Chiropractic under the laws of any other state, territory or the District of Columbia having equivalent requirements and extending like privileges to practitioners of WV, may in the discretion of the Board be licensed to practice in WV without a written examination. 
​However, no license may be issued until the applicant has completed satisfactorily an oral jurisprudence exam, an interview with the Board members, and has paid the examination fee required by the Board.

  1. Completed Application (Application [reverse side] must be mailed to Chiropractic College for Seal and Signature of Dean, Secretary or Registrar.)
  2. Certified Birth Certificate - (Contact County in which you were born – Bureau for Public Health)
  3. Undergraduate College Transcripts
  4. Chiropractic College Transcripts
  5. National Boards (Parts I, II, III, & IV required)
  6. License Verification (If licensed in another state, active or inactive, contact State Licensing Board and request verification be mailed directly to the WV Board of Chiropractic)
  7. Non-Refundable Application Fee of $130 (certified check or money order payable to WV Board of Chiropractic)
  8. Signed & dated Notification of Conduct Prohibitions
  9. Criminal History Background Check:  www.identogo.com

​Temporary WV Licensure

Request AN Application for Temporary Licensure
  1. ​Completed Temporary Application
  2. Certified Birth Certificate - (Contact County in which you were born – Bureau for Public Health)
  3. National Boards (Parts I, II, III, & IV required)
  4. License Verification (If licensed in another state, active or inactive, contact State Licensing Board and request verification be mailed or emailed directly to the WV Board of Chiropractic)
  5. Non-Refundable Temporary Licensure Fee of $100 (certified check or money order payable to WV Board of Chiropractic)
  6. Signed & dated Notification of Conduct Prohibitions (attached)
  7. Curriculum Vitae

Temporary Licensure with Intent to Sit for Jurisprudence Exam

The remaining parts of the application packet must be submitted no later than 6 weeks prior to the jurisprudence exam:
  1. Completed Application for WV Licensure (Application [reverse side] must be mailed to Chiropractic College for Seal and Signature of Dean, Secretary or Registrar.)

​Pursuant to W. Va. Code §30-1-27, a person shall be granted an occupational or professional license, registration, or certificate if the person has been licensed or certified in another state, the license, registration, or certificate is in the same discipline and at the same practice level as the license, registration, or certificate for which the person is applying in this state and the person meets other conditions prescribed by W. Va. Code §30-1-27.
